if I read it right your skimmer is in 12" of water? That is way too deep. Find a way to prop it up and I think your problems will diminish. Also, to help with the air flow you can take a small drill bit and widen the hole where your airline goes. even a small amount of widening can improve skimmer function.

The Portland Water folks are using Chloramines in the water now, so therefore if you are not using the chloramine removing carbon filter in your unit, you are not removing all of the badness in the water.

Jeez, we almost sound like RC. It could be a bacterial bloom also. How much of the instant cycle stuff have you been adding? What test kit are you using for the DKH and Calcium? What do you dose? What buffer do you use for your DKH?

Don't clean your glass tanks with anything but white vinegar. Muratic acid will eat the silicone sealants. Every used glass tank I have ever set up got a cursory scrape of algae, then set right up. i think the material left helps with cycling. Muratic acid is good for cleaning acrylic tanks.

I wash mine twice on the whites cycle. No bleach, no cleaner, spin dry them in the washer, then hang dry them for a couple days.

How much does your temperature fluctuate? Does the tang have a good place to hide? Any other aggressive fish in the system? I have found the biggest cause of ich is temperature fluctuations. Even as much as a 2 degree change can cause stress.
